W-9 tax form 是什麼東西啊

註冊了一家網站，來了一封信，說是批准申請之前需要回答他的一些問題，還要傳真W9表格。
請問這些問題怎麼回答啊？還有W9怎麼填呢？

1.How will you get people to view these promotions ie get visitors to your site etc.?
3 U3 g0 i% x: K2. How many leads do you expect to generate daily or monthly?
3. Do you offer an incentive program or sweepstakes?  If so, please explain your program.
4. Please complete the W-9 tax form located on the following webpage and fax it back
